Medication Therapy Management Pharmacist Medication management, also called medication therapy management (MTM) pharmacist, is a patient-centred process to create treatment plans centred on each patient's medication-related goals. There are two distinct components to medication management: medication reconciliation and medication review. These two distinct processes are focused on preventing and resolving various MRPs. 1.Medication and medical problem reconciliation is the process of creating and maintaining an accurate medical problem and medication list that reflects all current active medical problems and medications the patient is taking and how they are being taken. 2.Medication review is the service whereby a clinician evaluates the medication list for appropriateness, effectiveness, safety, and convenience in conjunction with the patient's health status. Medication Management Medication management of SLE during pregnancy and lactation is challenging, as the potential benefits to the mother of any medication must be weighed against the potential risk to the developing foetus. Information on drug safety is often based on animal studies or case reports and the FDA safety-in-use category designations, A, B, C, D, and X, are often erroneously interpreted to suggest increasing risk with letter progression. The FDA is currently revamping its pregnancy labelling to more accurately reflect drug safety based on current knowledge. Medication Reconciliation Medication reconciliation is the process of comparing a list of the medication(s) a patient is taking to that which is ordered on admission, on transfer between areas of the hospital, and on discharge to ensure an up-to-date medication list.10 Medication reconciliation has become an important safety initiative in hospitals to prevent medication errors such as inadvertent omission or duplication of a medication, incorrect dosing, and drug interactions and to ensure that all health care providers can access a similar and complete medication list.

The importance of medication therapy management (MTM) in pharmacy practice Medication therapy management (MTM) is a service provided by pharmacists to eligible patients who participate in a Medicare drug plan, namely part D. The geriatric population, which is included among the eligible patients, is prone to taking numerous medications as they tend to have multiple disease states which lead to a plethora of medication-related problems. The goal of MTM is to optimize the therapeutic outcomes of an individual patient as well as to detect and prevent costly medication-related adverse events. Through a comprehensive review of all medications including their potential side effects, financial barriers, and how well a specific medication is working, pharmacists can help their patients come up with action plans to ensure they are getting the most out of their prescriptions. Pharmacists also often collaborate with their patients’ healthcare providers to optimize medication use for improved health outcomes. Medication-related adverse events are a major public health issue in the United States. The inappropriate use of drugs contributes to more than 1.5 million preventable, medication-related adverse events each year and accounts for an excess of one hundred and seventy-seven billion dollars in morbidity and mortality.
