Public & Non-Public Schools K-12 • Four or five doses of diphtheria-tetanus-pertussis (DTaP) • Two or three doses of Hepatitis B • Three or four doses of Polio • Two doses of measles-mumps-rubella (MMR) • Two doses of Varicella for kindergarten and grade one • One dose of Varicella, grades 2-7 • DOE Code 1: K-12 Immunizations are complete (Excluding 7th grade)

Data Element Changes Immunization Status • A new note added that reads: Beginning with the 2008-09 School Year, the only additional immunization required for students who meet Kindergarten immunization requirements and enter or attend the 7th grade is the tetanus diphtheria pertussis booster. • Slight wording changes in other notes

  20. Data Element Changes, cont. Vaccine Status, Date • This element has changes in doses and wording. • Also, it has been rewritten to make it easier to understand.

Reporting Reminders • Vaccine Status, Date - a local element and not reported to the state but it is used in records transfer and needs to be accurate and up to date so students can get into other schools. • Immunization Status – reported to the State in surveys 2, 3 and 5. • Survey 2 data are then sent to the Department of Health and need to be correct when reported in October.

Immunization Status • CODEDEFINITION • 1 – Complete Permanent immunization certificate documenting general immunization requirements • 2 - Temporary medical exemption requiring follow-up • 3 - Permanent medical exemption • 4 - Permanent religious exemption • 0 - Enrolled in this district fewer than 31 days or enrolled in any Juvenile Justice education program fewer than 31 days under Section 1003.22(5)(e), F.S. 8 - Permanent Immunization Certificate documenting seventh grade complete requirements • Codes 5, 6 and 7 are old now – these codes must have been given prior to the1993-94 school year to be reported.

Summary • Section 1003.22(8), Florida Statutes, and Rule 64D-3.046(3), Florida Administrative Code, require the Department of Health (DOH) to collect annual reports from all public and private schools to certify compliance with the compulsory school immunization law. The Department of Education and the Department of Health continue to work collaboratively to provide technical assistance and support to school districts and local health departments regarding the implementation of these requirements.
