Registration Details(If Taken earlier) (1) TIN No (2) CST No (3) IEC Code (4) Service Tax No (5) Central Excise Registration No (6) Entry Tax/Hotel & Luxury Tax/Entertainment Tax (7) Any Other Registration Details Documents Required: - 1. 1 Photograph of Proprietor 2. Pan Card copy 3. Residential Address Proof (Voter Card/Adhaar Card/Passport) (any one of these)

  2. 4. Office Address Proof: - a. Utility Bill (Any one from below) i. Electricity Bill ii. Gas Bill iii. Property tax receipt iv. Any Other Utility Bill b. Ownership Proof (Any one from below as applicable in your case) i. Electricity Bill (If owned and bill is on proprietor name) ii. Property Paper (If owned) iii. Rent Agreement (If taken on rent) iv. Consent letter (if taken on consent-for family members or owned but electricity bill is not in proprietor name) 5. Bank Details* Note: - *Required After registration of firm, for bank account details, a copy of cancelled cheque or extract of passbook/bank statement (containing the first and last page)
