Dementia has had the highest profile politically in decades. Everybody’s Business, Our Health our care our say forms a broad framework for changing services for people with dementia and strengthening involvementNational Drivers are requiring us to work in a more integrated way.     3. The Process Two workshops 2006-2007 organised by Bob Bolas, deputy chief executive SMHP. Key members of partnership organisations invited to attend. Representation was as wide as possible. Working Sub group brought together key ideas and balanced them against national drivers Distributed for extensive consultation.

    4. The Pathway Shifts the balance towards preventative services Partnership document for positive change Clarity of roles Support in the right place at the right time along the way Better care at the end

    5. Key Themes A personalised service Support for healthier independent lives Draws from National Evidence Promotion of remaining strengths Improved support for people with High level needs People in control by provision of information Recognising experience and strengthening involvement

    6. Progress to date There is a broad consensus across the partnership organisations. There has been variable commitment. Coincided with the national strategy and the Darzi clinical pathways groups. The document is a reference guide in some organisations, not all.

    7. Lessons learnt There should have been a steering group. A whole system approach was to big for all to comprehend. There was too much information. People have been unable to see the links between service lines. We should have avoided service examples. Did we allow people to continue thinking in silos?

    8. Next Steps A series of meetings are planned breaking down service lines into primary and secondary care and social care. Rationalise the pathway against the Darzi clinical pathways and the national strategy.
