  1. Picasa Syrup Picasa Syrup is a medicine that is used in the treatment of invasive Candida and Aspergillus infections in patients who are severely immunocompromised. These can include patients with HSCT recipients with GVHD or those with hematologic malignancies along with neutropenia that got prolonged from chemotherapy. Patients who are hypersensitive to the drug shouldn’t be prescribed this drug. This drug is used for the treatment of fungal infections caused by the Fusarium family. This drug is used for these infections when the state couldn’t be improved by treatment with amphotericin B. It is also used to treat fungal infections and conditions called chromoblastomycosis and mycetoma. Like the previous case, this drug is prescribed when such conditions could not be improved by itraconazole. This drug is even used to treat Coccidioides when the treatment from amphotericin B, itraconazole, and fluconazole failed to make the condition better. Picasa Syrup works by inhibiting the synthesis of ergosterol. It does this by binding and inhibiting lanosterol-14alpha-demethylase, which is present in almost all fungi. This component is a vital part of fungal cell membranes. Hence causing an antifungal activity. Common side effects of the drug include diarrhea, nausea, vomiting, coughing, headache, fever, and hypokalemia. Severe side effects include hypersensitivity to the drug, herpatic toxicity, QT prolongation, and Arrhythmias. This drug should NEVER be coadministered with HMG-CoA reductase inhibitors as it raises the plasma concentration of this drug and hence increasing the chances of rhabdomyolysis.
