Things to Remember • Customize your Super Kit • Add your logo and/or contact information to: • Bottom of opening Super Kit page • First 3 Sheets in the Broker Kit • Group Applications in the Forms Pack • Delete the Group Applications for states in which you do not do business (in the Forms Pack) • Customize your JLT Transmittal Form • Add your name and contact information to the master form

Things to Remember (cont.) • Agents return their cases to you (GA) for scrubbing and forwarding to JLT • Use the Transmittal Form to JLT Services • Check Input sheet in Broker Kit for correct zip code for dental rates • Ensure that Voluntary rates are used if the case is Voluntary • Is the agent appointed? • Are the Group App and Enrollment forms complete? • Is there a check for 1 month’s premium payable to Security Life Insurance Company of America • Will this reach JLT Services by the 20th of the month preceding the effective date?
