SPECIAL DIABETES PROGRAM FOR INDIANS Diabetes Prevention and Healthy Heart Initiatives: Year 3 Meeting 1 • November 2010 - Year 1 Meeting 1 • Introduced to the idea of dissemination and online toolkits • Discussed what an Initiatives toolkit should contain • February 2011- June 2011 • Reviewed four online toolkits • Reviewed 2 Initiatives toolkit draft outlines of 8 basic modules each • Provided list of tools, materials, resources, & strategies for each module • Compiled lists into 2 detailed outlines • Revised detailed outlines (2 small work groups of program staff)

SPECIAL DIABETES PROGRAM FOR INDIANS Diabetes Prevention and Healthy Heart Initiatives: Year 3 Meeting 1 • July 2011- March 2012 • Provided tools, materials, and resources for each module based on detailed outline • Organized all materials into packets by module • Reviewed packets and rated items (Y1M2 & Y2 M1& 2) • March 2012 – September 2012 • Prepared PDF portfolios of tools and materials rated one overall (CC) • Prepared Documented Quote for website development • Reviewed Module 2 – Recruitment & provided input (Y2 M3) • Listed facilitators and barriers to dissemination and adoption (Y2 M4)

SPECIAL DIABETES PROGRAM FOR INDIANS Diabetes Prevention and Healthy Heart Initiatives: Year 3 Meeting 1 • October 2012- May 2013 • Began working with website developer • Prepared packets & guidelines for writing groups • Wrote first draft of 16 modules based on detailed outline of toolkits and input collected during Year 2 Meeting 3, June 2012 (6 writing groups) • Began gathering missing materials for appendices • Wrote first draft of 2 modules (Documenting Activities & Outcomes) • Reformatted 2 modules for sharing

SPECIAL DIABETES PROGRAM FOR INDIANS Diabetes Prevention and Healthy Heart Initiatives: Year 3 Meeting 1 Next Steps – continued • Ongoing Toolkit Website Development 3-4 months • Populate website with modules, appendices, links, video links • Beta test (subset of grantees) • Evaluate format, content, delivery, and relevance • Revisions, based on evaluation results for Final Review • Final Steps 2-3 months • Form Resource Bank (subject experts from among program staff) • Develop and implement plans for national launch • Provide technical assistance (TA) for implementation and coordinate TA by Resource Bank
