  2. Linkages AKA “Cross-over” • Initially known as “Cross-over” the 1999-2004 Contra Costa Plan involved: • Cross-over training for CWS & Cal WORKs Staff • Identification of “Crossover” cases • Coordinated services

  3. Goals of Linkages in Contra Costa • Child and Family Safety • Improving Service Delivery • Meeting basic needs • Eliminating ‘system barriers’ • Providing access to resources/assistance • Ensuring that our services are fair and equitable • Healthy families moving toward stability & self-sufficiency

  4. Linkages at Intake: CFS • If Linkages cases are identified by clerical staff during the clearance process, • The case folder is flagged with a bright green form. • The date, case name, and case number are entered on the “Linkages Log.”

  5. Linkages Intake: CFS, continued • Referral is assigned to a District Emergency Response Social Worker. • This worker records in the case narrative that Linkage to a CalWORKs case exists. If an investigation concludes that a case should be opened, • Clerical staff updates the case flag and • SW codes the Special Projects screen in CWS/CMS.

  6. Case Plans • A referral is sent to the Welfare-to-Work Assessment & Intensive Services (AIS) Unit. If WTW services are available to the parent(s), the AIS worker contracts the CFS worker to recoup of the referral. The AIS worker then schedules a meeting with them to create a WTW Case Plan. This meeting is held within 2 weeks of the referral. • In advance of the Case Plan meeting, the referral form is returned to the CWS Social Worker, advising him/her of the availability of WTW services and the date and time of the of the Linkages Assessment Meeting, or “LAM.” This meeting is scheduled to be held within one month of the referral.

  7. Case Coding • When notified of the availability of WTW services for a CWS-involved family, the CWS Social Worker codes the case as an AB 429 case on the Special Projects screen in CWS/CMS. • AIS Code

  8. The LAM The Linkages Assessment Meeting, or LAM, is convened within two months of the referral in order to provide additional resources to the family, acknowledge successes and note outcomes.
