SUBJECTIVE • 6 year old male , admitted April 2009 • cough • 2 weeks PTC:intermittent early morning sneezing , occasional watery nasal discharge • 1 week PTC: occasional non-productive cough, but otherwise well • 3 days PTC: violently coughing, dyspnea while playing • tapped at the back; coughing became less severe and dyspnea disappeared after a couple of minutes • able to resume playing • continued to have bouts of “croupy” cough, non-productive, with occasional episodes of difficulty of breathing • eat well and do regular activities

  3. (-) weight changes • (-)lymphadenopathies • (-)cyanosis • (-)chest pain • ear, cardiac, abdominal findings are non-remarkable • (+)history of asthma and allergic rhinitis; relieved by salbutamol • (+) allergy to dust • (+)frequent sneezing in the morning • (+)family history of asthma • Up to date for age immunizations

  4. OBJECTIVE • Obese • mild respiratory distress • tachypneic • tachycardic • low grade fever • boggy violaceous colored inferior nasal turbinates • symmetrical chest expansion • mild intercostals retractions • decreased breath sounds (R) • ronchi and wheezes
