  2. The Crystal Ball • If you had a crystal ball, what would you see? Can and will your child work? Who will be the support system when the ISD is gone? Where will your child live, if not with you? How will services be funded? Transportation? The school bus is no longer an option. How will your child continue to learn? College? Agencies? On-the- job?

  3. The Transition Planning Process • Planning the path to the dream: • First and foremost, we must obtain quality information about the young adult with a disability and the post-school goals. STUDENT Person-Centered Thinking OTHER PARENT/GUARDIAN

  4. Planned Positive Outcomes • Transition is a series of building blocks during the secondary years to facilitate transition to the post-school goals. • Neither the school, student, or parent/guardian can plan or implement transitions services alone. • It takes all 3 school-based partners. • Then add, Community and agency support

  5. Uninformed Partners • If students are trained in self-determination and Student-led ARD/IEP Meetings, how can they continue to meet the post-school goals after high school? • If parents are trained and informed/involved about transition services, how can they be partners in the process and carry-on the support after high school?

  6. Paperwork or Process • Transition Services is NOT a form to complete in the IEP and during the ARD Meeting. • There must be follow-up: • Annual goal that facilitates transition. • What is the student doing to help him/herself? • What is the parent doing to facilitate the process?

  7. All Parents Have A Responsibility • Parents of children with and without disabilities have a responsibility for “transition”.

  8. Unintended Outcomes • Can be prevented • Waiting till the final year to make decisions. • Waiting till after high school to make decisions. • Not seeking information, asking questions, taking an active part in the ARD and transition planning. • Unexpected • Not considering all options. • Not having a Plan B, should A not work. DEVELOP A VISION FOR THE FUTURE

  9. Poorly Designed Services • What are the roles of each member? • Role of the school • Role of the student • Role of the parent • Where the roles fulfilled?

Poorly Designed Services • Transition Planning and Services: • Parent Input • Parent Input prior to the ARD Meeting and not “DURING” the ARD Meeting. • Discussion of student and parent input in the transition services planning. • Actions for all members: school, student, parent. • Review of transition services to determine on-going results and needs to facilitate transition services.

  11. Preventing the Nightmare • Parents have more of a role than they realize or think! • What is your worst “nightmare” for your child? • Have you shared this during the transition planning? • Are transition services discussed that are designed to prevent the “nightmare”?

  13. Things to Think About • Have you completed a Parent Survey for Transition Services? • How much longer before your child exits public school? TIME IS IMPORTANT!! • What are your child’s post-school goals? • What are your post-school goals for your child? • What needs to be done?
