Deployment Health Assessment Program • Deployment Health Assessment Program • The purpose of the DHAP Program is to screen and review each service member's current physical health, mental health or psychosocial issues commonly associated with deployments, special medications taken during the deployment, possible deployment-related occupational/environmental exposures, and to discuss deployment-related health concerns. Positive responses require use of supplemental assessment tools and/or referrals for medical consultation. The provider will document concerns and referral needs and discuss resources available to help resolve any post-deployment issues. The Deployment Health Assessment forms are to be completed by the service member and reviewed by a provider within 60 days prior to deployment, 30 days after redeployment, and 90-180 days after redeployment.

  5. Pre Deployment Health Assessment • Pre Deployment Health Assessment (DD2795) • The Pre-Deployment Health Assessment is required to be taken within 60 days of deployment. The purpose of the Pre is to gauge the physical and mental health of the service member before actual deployment

Post Deployment Health Assessment • Post Deployment Health Assessment (DD2796) • The Post Deployment Health Assessment must be scheduled for a face-to-face health assessment with a trained healthcare provider within 30 days after returning to home or processing station. The provider will document concerns and referral needs and discuss resources available to help resolve any post-deployment issues.

Post Deployment Health Reassessment • Post Deployment Health Reassessment (DD2900) • The Post Deployment Health Reassessment must be completed within a 90-180 day window after returning to home or processing station. This is the last screening for any health issues in the Deployment Health Assessments. This screening can be completed with a provider face to face, or depending on the circumstances it can also be completed over the phone. The provider will document concerns and referral needs and discuss resources available to help resolve any post-deployment issues.

  11. Deployment Health Assessments Service members access their DHA’s through their My Medical Readiness in AKO , then click the Deployment Health Assessments link

  12. Medical Health Assessment (MHA) Service members complete their Health Assessment forms in MHA. Providers also access MHA to sign the forms.

Applications for Tracking DHA’s Information on tracking and compliance of the DHA’s can be found in these applications • MEDPROS – Medical Protection System • Contains the reports to track compliance and completion percentages as well as referral statistics • MHA – Medical Health Assessments • Where the DHA forms are completed by the service member and signed by the provider • MWDE – MEDPROS Web Data Entry • Contains the Priority Report

  15. MEDPROS The acronym MEDPROS stands for Medical Protection System. MEDPROS is the Army’s automated database designed to meet Department of Defense requirements in maintaining Unit and Individual Medical Readiness. It is designed to provide commanders with a real-time, world-wide operational system to manage the Medical Readiness and deployability of their unit. MEDPROS provides commanders at all levels with the capability to track medical and dental readiness by Unit Identification Code (UIC), individual SSN and/or task force. MEDPROS captures data for Soldiers in all Army Components – the Active Army, National Guard, and Army Reserves, DA Civilians, DA Contractor personnel – and all sister services.

MEDPROS Reports • Pre Deployment Report • Displays compliance status of the Pre-DHA (DD2795) • Post Deployment Report • Displays compliance status of the PDHA (DD2796) • PDHRA Report • Displays compliance status of the PDHRA (DD2900) • PDHRA Command Drill Down Report • Displays PDHRA compliance status by UIC, allows user to drill down UIC trees to view specific commands • ARNG by State Summary report • Displays PDHRA statistics filtered by State • Aggregate Reports PDHRA/PDHA • Displays referral statistics based off the PDHRA and PDHA forms

  18. MEDPROS - PDHRA Report Output includes by name roster and summary about the selected population.

  19. MEDPROS - PDHRA Command Drilldown Report Allows you to drilldown through the UIC tree structure viewing PDHRA compliance at each level.

  20. MEDPROS - PDHRA Command Drilldown Report The ARNG by State Summary displays PDHRA compliance status totaled by State.

  21. MEDPROS - PDHRA Aggregate Report Options The PDHRA Aggregate Report allows you to view referral statistics based on selected criteria. This does not reveal any personal data.

  23. MHA – Medical Health Assessments The MHA application is a repository for the DHA forms and contains the Referral Tracking Module

  24. MHA – Referral Tracking Module The Referral Tracking Module allows users to run reports based on referral statistics. The reports available are Individual, Aggregate, and Compliance Referral Reports

  25. MWDE Priority Soldier Report The Priority Report allows you to view all Priority and Non-Priority PDHRAs according to the above criteria. This can return Partial or Completed PDHRAs.

Deployment Health Assessments - DHA Upcoming Changes • Implementation of new DHA Forms • New versions of the DD2795, DD2796, and DD2900 will be implemented into MHA • DMDC Interface • MEDPROS will be receiving a new data feed from the Defense Manpower Deployment Center (DMDC) which will provide actual deployment dates for deploying service members • MEDPROS Reporting capabilities for the Pre, Post, and PDHRA will be expanded • New logic will be incorporated along with the DMDC Interface that will allow users to have a more overall complete awareness in tracking DHA compliance
