  2. PIP Indigenous Health Incentive A new Practice Incentive Payment (PIP) is now available to encourage general practices to provide better health care for Indigenous patients, including best practice management of chronic disease.

  4. Seek consent to register their Aboriginal patients Establish a mechanism for follow-up Undertake cultural awareness training Annotate PBS prescriptions for eligible patients • Sign-on payment • To receive the $1000 sign-on payment practices must agree to:

  5. Is aged 15 years and over; and Has a chronic disease (s) Has had (or been offered) a Health Check; and Has provided informed consent to be registered for the PIP Indigenous Health Incentive 2. Patient Rego Payment $250 paid to practices for each patient registered who:

Tier 1: Payments of $100 per eligible patient per year to have a care plan in place and at least one review (ie 721 and 725 in place), OR two reviews in place (where a care plan is not necessary that year) Tier 2: Payments of $150 per patient per year to provide the majority of care (not just chronic disease care) i.e. a minimum of 5 services per year. 3. Outcomes payment

  8. CDM Program The PIP Indigenous Health Incentive will act as a gateway to other measures under the Closing the Gap initiative, including the Chronic Disease Management Program: Care coordination and other supplementary services PBS co-payment relief

  9. MBS Item 715 For all Indigenous people Focused on early detection, diagnosis and intervention for common and treatable conditions Minimum billing every 9 months Health Checks

Additional Practice Nurse and registered Aboriginal health worker services – item 10987 • Maximum of 10 services per calendar year • Patient must have had a Health Check (Item 715) • Used to provide:  • - Check on progress and service access • - Interventions, education, monitoring, counselling & lifestyle advice

  11. Provides cheaper or free medications to Aboriginal patients Only for PIP practices, and Metro and regional AMSs Patients must give consent and registration is required (separate to the PIP registration) PBS Co-payment Measure

Patients must identify as Indigenous, and Have a chronic disease or chronic disease risk factor, and In the opinion of the doctor: - would experience setbacks if the person did not take the prescribed medicine, and - are unlikely to adhere to their medicine regimen without assistance through the Measure PBS Co-payment Measure - eligibility

Integrating CTG with other programs in your Division • Incorporate Aboriginal Health into mainstream Division program activity – you are not the sole person responsible for it • Aboriginal Health - standing agenda item at both Board and Division staff meetings - cultural awareness training for all Division staff • Determine if other Division programs address Aboriginal Health issues – ANSC; Immunisation; Diabetes, Lifestyle Modification; CDM; Mental Health; ACAI etc • Aboriginal Health as key element of practice support role – Accreditation, IT/IM, APCC • Include Aboriginal Health into every CPD activity across all programs – case studies, updates • Promote through Division website, newsletters, faxouts, emails etc
