  1. Service utilization patterns of prisoners, homeless and uninsured residents as evaluated through community wide data sharing Session 5155.1 Wednesday November 8 American Public Health Association (APHA) Annual Meeting Boston, MA Mary M. Crimmins MA, CPEHR, CPHIT and Katherine L. Cauley Ph.D. Wright State University Kathleen Shanahan, Dan Foley and Shawn Dunlavy Montgomery County Ohio

  2. Learning Objectives • Articulate strategies and best practices to analyze data shared across organizations. • Specify the data elements that can be used as indicators of services utilization. • Identify potential benefits to clients/patients that result from data sharing in a web based environment.

  3. 3 Information Systems • CJIS Criminal Justice Information system • HMIS Homeless Management Information System • HIEx™ HealthLink Information Exchange

  4. CJIS Criminal Justice Information System • Montgomery County Clerk of Courts • Public records from 12 court systems • Aggregation of the data creates ethical questions about the use of the data • Access questions have to be answered • Data currently used by law enforcement and child welfare

  5. HMIS Homeless Management Information System • Shelter Policy Board is a coalition of homeless serving organizations administratively housed in United Way • System was mandated by HUD • Proprietary product of Bowman Internet Systems • Used by multiple non-profit organizations in the homeless serving agencies

  6. HIEx™ HealthLink Information Exchange • HealthLink is a coalition of health related organizations administratively housed at the Center for Healthy Communities at Wright State University • Cross sector (health & human services) • Household record for eligibility & means tested programs • Patient centric health record • Open source philosophy

  7. Data sharing • Proof of concept is the first step • Create a unique identifier for all individuals from an algorithm • Apply that algorithm to the data in all 3 systems • Potential of interoperability through web services

  8. Results • Homeless (HMIS) and Criminal Justice (CJIS) sampled individuals with the highest jail use rates and found chronic homeless use of the jail was high. • The court system has responded with a special court for behavioral health and homeless

  9. Results from HIEx™ • Most of the 26,000 individuals with data in HIEx™ live in 9600 households. • The focus of the program has been to improve access to health care for families and children. • At least 10% of children enrolled in Dayton Public Schools already have data in HIEx™
