Project Process PROJECT PROCESS CURRENT STATE Baseline data gathered ASSESSMENT Needs identified Action areas identified to progress integration within the project remit CONSULTATION AND Development of ‘integration components’ in DESIGN consultation with key personnel TRIAL AND FEEDBACK ‘Integration’ in place/ changes implemented Opportunity for refinement **FINAL REPORTING** Repeat of staff surveys Repeat of ‘Pathway Review’ Recommendations

Issues identified... • Employability is not given priority at clinical practice planning level • Employability is not given priority at strategic planning level within the MCN • Health professionals are not fully aware of the resources available to them • Health professionals are not necessarily transferring their theoretical knowledge • into clinical practice • Patients are not getting consistent information and support regarding • employability issues

What could ‘Improvement’ look like? • Changes to standard documentation • Changes to the format of specific patient contacts • Electronic resource for staff to access • Inclusion of health and work information within existing resources used • Staff awareness of how to find information • Acknowledgement of health and work agenda within the overarching work of the MCN • Annual objectives • Future initiatives/ developments • Recommended training

Recommended action area: Embed Health and Work at Planning Level Embed Health and Work at Clinical Practice Level Improve Knowledge/ Awareness around Health and Work Health and Work within 2012/13 MCN Objectives Process in place for improved Referral to OT for VR support- HAW links included within MCN Website- Available to staff and patients Development of W.O.R.K. Brief Intervention Amendment of key clinical documentation Supplementary patient leaflet Electronic resource- Hi-Net Commitment from MCN to promote and encourage HAW related training and development to respiratory staff OUTPUTS In addressing the original Recommendations from the ‘Current State Report’ (July 2011), the following has been put in place:

Impact to date • Planning Level • Commitment from MCN to retain H&W • Commitment from MCN to promote H&W training • Practice Level • Increase referrals to OT from Respiratory Services • Increase in signposting to external organisations for support around employability • Staff Knowledge/ Awareness • Dramatic increase in survey respondents • Numbers accessing Hi-Net increasing • Ad-Hoc requests for info etc • Interest in further training around Health and Work
