  5. Peer Counseling • Based on the belief that a counselor with shared experiences is the best guide to independence: • Provides motivation as a role model • Assists with self-exploration • Provides information on community resources • Advocates and promotes community change

  6. Peer Network • Provides opportunities for individuals with disabilities to explore common issues and share strategies for success: • Peer Support Groups

  7. Independent Living Skills Training • Participants determine training programs to gain individual skills and foster community involvement • Disability Rights • Self-Advocacy/ Housing Options • Transportation Options • Budgeting • Recreation options

  9. Community Based Waiver Services Waivers are optional programs that afford States flexibility to develop and implement alternatives to institutionalization. To determine eligibility for a Waiver, criteria for Medicaid eligibility must be met. You will then be screened to determine if you need the level of care provided in an institution.

  10. Virginia Waivers Currently in the state of Virginia there are 7 Medicaid waivers • AIDS/HIV • EDCD (Elderly or disabled with consumer direction) • DD (developmentally disabled) • MR (mentally retarded) • Technology Assisted • Day Support • Alzheimer’s

  11. Case Management for the DD waiver Case managers work with individuals to develop an individualized Plan of Care. Case managers coordinate services and supports within the community and monitor those services to ensure the Plan of Care continues to meet the needs of the individual. Service Facilitation for the EDCD waiver A service facilitator is responsible for identifying the needs of the individual, developing a Plan of Care to meet their needs, and ensure that the individual understands his/her rights and responsibilities in the program.
