Causes of proptosis • Most common cause for unilateral and bilateral proptosis is “Thyroid eye disease”. • Other causes: Orbital infectious cellulitis. Inflammatory psuodotumour. Orbital tumours. Lacrimal gland tumours. Trauma.
Classification Anatomical Clinical. Aetiological. Pathological
Anatomical Classification Anterior. Intermediate. Posterior. Panuveitis.
Anterior segment complications of blunt trauma Vossius ring Hyphaema Iridodialysis Sphincter tear Cataract Lens subluxation Angle recession Rupture of globe
Posterior segment complications of blunt trauma Choroidal rupture and haemorrhage Avulsion of vitreous base and retinal dialysis Commotio retinae Macular hole Optic neuropathy Equatorial tears
Episcleritis Superficial inflammation of the sclera
Intraocular Tumours • Primary or Secondary (metastatic) • In children the most common is Retinoblastoma. • In adults the most common primary malignancy is Malignant Melanoma
Presentations of retinoblastoma • Leukocoria - 60% • Strabismus - 20% • Secondary glaucoma • Anterior segment invasion • Orbital invasion • Orbital inflammation
Increase of the intraocular pressure above normal limits with affection of the visual fields and optic nerve head
