  1. INTEGRATINGTHE HEALTHCARE ENTERPRISE RADIATION ONCOLOGYIHE-RO Bruce Curan, IHE Radiation Oncology Technical Committee co-Chair

  2. IHE-RO: The Concept Hell is a place where nothing connects. - T S Eliot

  3. IHE-RO Participants • Radiotherapy society participants include: ASTRO, AAPM, ACR, AROI (India), CARO (Canadian), CSRO (Chinese), ECS-RO (Egyptian), ESTRO (European), JASTRO (Japanese), RSNA • Equipment manufacturers include: BrainLab, CMS, Elekta, IMPAC, Mirada Solutions, Nucletron, Philips, Siemens, Tomotherapy, Varian • Others: ATC, IAEA, NCI, NEMA

  4. IHE-RO Prabhakar Tripuraneni MD Jatinder Palta PhD Planning Comm. Jay Cooper MD Keith La Plain, Varian Technical Comm. Bruce Curran PhD Stuart Swerdloff, Impac

  5. IHE-RO Goals for 2006-7 • Information Exchange Between: • CT Scanner • Contourer • Geometric Planner • Dosimetric Planner • Dose Displayer COMPLETED

  6. IHE-ROIn Practice

  7. IHE-RO 2006-7 Informal Connectathon

  8. IHE-ROTechnical Framework Vol. 1-2 • RT Objects Integration Profile The integration profile for 2007 involves the flow of DICOM images and treatment planning data, from CT scan through dose display, for 3D conformal, external beam radiation therapy. The emphasis for this first Integration Profile is on reducing ambiguity and facilitating basic interoperability in the exchange of DICOM RT objects.

  9. Proposed Use Cases2007 - 2008 • Scheduling Treatment Delivery and Verify/Record  • Residual Dose Optimization • Image Registration • On-line Image Review (reference image vs. delivery image) • Compositing dose

  10. CONNECTATHON 2008 • Calendar • Feb 2007 Decision on 2008 content: • Jan 2008 Informal Connectathon: • August 2008 Formal Connectathon: • ASTRO 2008 Formal Demonstration: • Connectathon Content • Must include treatment delivery based on DICOM Supplement 74 • Need to prioritize other contents

  11. Dose Prescription:Electronic Standards to be decided • All treatments start with a prescription • Reduce & eliminate ambiguity • Quantifying : exactly, less than, more than, no less than, no more than, units, target, target coverage • Used for • Targets • Organs at risk (OAR) including limits • Functioning organ constraints

  12. Future Directions • Brachytherapy (LDR & HDR) • Clinical Trials Submissions • 4D : gating, breath hold, adaptive, fluoroscopic monitoring • Plan transfer to treatment delivery device • Enhanced dose exchange • Enhanced work list : Dose Calculation Scheduling, Planning Scheduling (Simulation, Contouring, Planning) • Multiple-independent delivery devices – Micro MLCs • ION Support
