Next Steps – Part 1 (QI project) Complete Learning Session Evaluation Form Complete Travel Expense Form and return to Lindsay Wilson to arrive no later than September 30, 2011 Claim CME Credit (Tab 3) Live Learning Session: 21 days after the session Performance Improvement: After 1 data cycle is complete (staff will email you) Prepare practice Begin monthly data collection: Data cycle opens October 15 and closes October 30 Submit first 10 charts of Medicaid, Medicaid health plan, or KidCare via EQIPP Submit Monthly Progress Report via Survey Monkey Links to data collection requirements will be available on the Project Workspace Web site and will be emailed to teams each month Participate on Monthly Conference Calls (Practice members only)

Monthly Conference Calls/Webinars Plan to attend ONE call per month Agenda, Slides, and Webinar information will be sent out prior to call Third Wednesday of every month at 1 pm ET Oct 19, Nov 16, Dec 21, Jan 18, Feb 15, March 21 OR Fourth Tuesday of every month at 12 pm ET Oct 25, Nov 22, Dec 27, Jan 24, Feb 28, March 27 Reminder: these calls count toward the “meeting” requirement of MOC Part 4 credit!
