PFA Medial Clearance Process to ensure members receive appropriate screening/clearance prior to participating in the PFA PFA medical clearance has 3 components: PHA PARFQ Pre-physical Activity Questions No member is authorized to participate in the PRT portion of the PFA without being clear via all 3 components. The in-processing medical screening for new accessions is sufficient for the PRT when students are not required to complete a PHA until their first duty station. PFA Medical Waiver Authorized for members with injury, illness or for members recovering from a surgical or medical procedure Must be granted prior to the PFA May only be issued on NAVMED 6110/4 NAVMED 6110/4- PFA Medical Clearance/Waiver form. Medical Waivers (cont’d) CO has final approval on all PFA medical waivers Members using Tri-care remote must provide the civilian provider with a NAVMED 6110/4 for recommendation of a PFA medical waiver. Waiver request must be approved via their military chain of command. Members serving in Joint environment may obtain medical waiver approval via their authorized forms and procedures. Medical Evaluation Board Eligibility MEB eligibility will activate if member is granted an additional medical waiver after 1 July 11 Not all PFA waivers will make a member eligible for a Medical Evaluation Board (MEB) Members with previous medical waiver prior to 1 July 11 are safe as long as they are not granted an additional waiver after 1 July 11. MEB Eligibility #3 CO may request a MEB when a member’s fitness/health is questionable for continued Naval Service or mission readiness The CO may request a medical record review of a member who is otherwise not eligible for a medical record review due to only being waived from 1 PRT event.
