  1. Power Point Presentation Template for CNW 2012 The slides shown should be used as a template for your power point presentation. The information included is necessary to ensure A.S.P.E.N. remains in compliance with various accrediting bodies. Additionally, the A.S.P.E.N. logo should be on your slides. Refer to the speaker guidelines to review the required accreditation information. NOTE: The slide titles used are just examples, it is not necessary for your presentation to include these exact titles.

  2. Presentation Title Slide Full Name, Credentials Affiliation Affiliation City and State Note, in order to comply with accreditation guidelines, your name, credentials, affiliation, city and state must be included in your presentation.

  3. Disclosures • Bulleted or numbered text Note, in order to comply with accreditation guidelines, disclosures should be included in your presentation. For any disclosures you must list the company and your affiliation with the company

  4. Discussion of Off-Label Product Use • If your presentation will include the discussion of off-label product use, please inform the audience of this in the beginning of the presentation.

  5. Learning Objectives • Three (3) learning objectives Ex: Upon completion of this session, the learner will be able to….. Note, in order to comply with accreditation guidelines, learning objectives should be included in your presentation.

  6. Summary or Outline • Brief summary or outline of presentation Note, in order to comply with accreditation guidelines, a brief summary or outline should be included in your presentation.

  7. Text, applicable charts, tables or graphs This information is not required, but can be visually stimulating for the audience. This is considered the descriptive part of the presentation and may be several slides in length. Note, tables and graphs should fit within the slide

  8. Learning Assessment Questions • Numbered text. Minimum of three questions, (multiple choice, true/false or case study) • You can choose whether or not to include these questions and answers in the beginning, end, or both – whichever is best for your presentation

  9. Learning Assessment & Post Test Answer Key • Numbered text. Answer key should include rationale and references Note, in order to comply with accreditation guidelines, a learning assessment answer key should be included in your presentation.

  10. References • Numbered Text. Current references to support your overall presentation Note, in order to comply with accreditation guidelines, references should be included in your presentation.
