Process Underway for AE Changes • Change in intent and purpose of AE • Initial meeting with Corinda • Meet with legal to determine requirements of AE • Name change to “Annual Evaluation and Progress Report” • Met with Jennifer Rosenbaum & Angelia to discuss POMP and ways to incorporate performance measures into the AE • Met program by program with content experts to discuss pieces of the AE • Draft, review, and edit among LPO leadership • EMT discussion (December 4) • Final draft Dec 31 • Review and revise (January) • To IT in February

HISTORY: The current Annual Evaluation (AE) model was developed by the 2000 AE workgroup, where the content reviewed in the Guide was to reflect requirements of Federal or State legislation or regulations. This AE is for information gathering /verification purposes only; a look back at the previous 365 days.

  5. GOAL: To transition the AE from information gathering/verification to performance measures that align with the goals set out in the 2015-2019 State Plan on Aging. Each of the programs could be evaluated to determine how the actions of the AAA align with the State Plan, and ultimately support the mission and vision of NYSOFA.

  6. Our Mission is to help older New Yorkers be as independent as possible for as long as possible through advocacy, development and delivery of person-centered, consumer-oriented, and cost-effective policies, programs and services which support and empower older adults and their families, in partnership with the network of public and private organizations which serve them.

  7. Meeting Notes • “Parking lot” on the wall will be used for issues that have not been resolved within the time frame
