Travel Basics • All business travel reservations should be made through American Express • Access will be granted to Cliqbook by Friday • All air travel class is coach • Travelers must use preferred carriers whenever possible • Westin Galleria and Sheraton of Framingham are our preferred hotel vendors • Should choose the lowest logical fare possible • Please refer to the Acadia T&E Policy, Version 2.0

Expense Basics • All airfare booked through American Express will be direct billed to Acadia • AMEX Corporate Cards should be used to cover all other expenses • Applications are available in eRoom • Expenses should be submitted within two weeks from completion of trip • Expense form available in eRoom • Expenses should be forwarded to expenses@acadia.comafter receiving manager approval in e-mail and will be reimbursed with your next available paycheck • Seconded employees should submit their expenses through their respective parent company system

Payroll • Acadia pays bi-weekly, 26 pay periods a year • Pay is one week in arrears • All benefit deductions are based on the 26 pay periods. • Direct deposit only, however, until setup paycheck is live • When you receive your first pay statement, go to www.paystatements.adp.com and click “Register Now” • Information on first payroll statement should be used to register for receiving your online pay statements

Disability Plans: Long Term Disability (LTD) • Pays 66 2/3% of monthly benefits salary up to $5,000 • Automatically enrolled • 13 week elimination period • Cost covered by Acadia • LTD Buy-Up Option • Benefits salary over $90,000 • Automatically enrolled • Benefit remains at 66 2/3%, with an increase of up to a $15,000 monthly maximum • Bi-weekly employee cost based on benefits salary is shown in your My Acadia Benefits booklet If you waive this coverage, proof of insurability will be required for this coverage in the future.

Life Insurance: Coverage Basic Group Term Life - up to $1,500,000 maximum; employee cost based on benefits salary Accidental Death & Dismemberment (AD&D) - no employee cost, up to an additional $1,500,000 maximum Business Travel Accident - no employee cost, principal benefit $300,000 (may vary) Must complete Beneficiary Designation Form.If you waive this coverage, proof of insurability will be required for this coverage in the future.

How to Enroll • Fill out Benefits Enrollment Form in packet • Scan and email benefits forms to benefits@acadia.com • Coverage begins TODAY for all benefits except Medical – effective the 15th of the month • Cost of COBRA(medical) will be reimbursed should you need it and will be prorated from your start date to eligibility date. Must sign up for medical plan to be eligible. • Cards can take 4-6 weeks to receive; policy numbers available in the eRoom in the interim
