Dental Benefits* Oral & Maxillofacial Surgery • Surgical removal of impacted/un-erupted teeth and buried teeth (tooth/teeth must be symptomatic or causing damage to an adjacent tooth) • Surgical removal of complicated buried roots (roots must be diseased or causing symptoms, or flap of gum is raised and bone is removed) • Enucleation (removal) of cysts of the jaw • Treatment of cancers • Temporal Mandibular Joint (TMJ)

  4. Claims Process Inpatient, Hospitalisation & Surgery Call the 24-Hour Assistance Hotline: 1800 880 8181 or +65 6880 8181 (out of Singapore) Provide the following details: Policy Number, Name of Patient, Date of Birth Name of Hospital, Date of Admission, Attending Doctor/Surgeon

Claims Process Inpatient, Hospitalisation & Surgery Logon to: expatinsurance.com.sg/reflectgeo Download & Complete Pre-Authorisation Form Upon approval, AXA will arrange for direct settlement with the Hospital/Provider. *For ease of assistance with Payment Guarantees, consult the International Directory of Hospitals and utilise a preferred provider

  6. Claims Process Outpatient Claims Two Options: Visit a Cashless Outpatient Clinic in Singapore Visit a Doctor of Your Choice

  7. Claims Process Cashless Outpatient Claims Visit a clinic on the Cashless Billing List Present your medical card at time of visit The clinic will bill AXA directly

  9. Claims Process Outpatient Reimbursement Claims AXA will credit your preferred bank account in the currency of your choice. *Note: All claims must be submitted within 60 days of treatment. Timeline: Upon receipt of claim form and receipts by post, AXA will process within three weeks time.

  10. Coverage for your Family Reflect Geophyscial will pay 100% of employee premiums. Offshore Employees have the option to add their Spouse and/or Children* to the plan at rates subsidized 50% by Reflect Geophysical. *Children are eligible up to Age 21

Adding your Family • Two deadlines for addition of your dependents: • 16 January 2012 • 20 February 2012 Send a copy of each family member’s passport to: reflectgeo@expatinsurance.com.sg You will receive a brief Health Declaration to be signed You will receive a confirmation including terms of coverage and medical insurance cards for each member *For Health Declarations received by 20 February, coverage will be back-dated to 1 November 2011 and claims settled accordingly
