Pediatric Discharge Wizard for Efficient Appointment Scheduling

This tool in HEO/Wiz helps providers facilitate discharges by notifying medical receptionists to schedule follow-up appointments before patients are discharged. It also provides printouts of discharge letters, instruction sheets, and asthma action plans.

Discharge Wizard Printouts • Patient Discharge Letter (1 copy) given to pt • Patient Instruction Sheet (1 copy) Nursing no longer needs to complete the handwritten Discharge Instruction Sheet. • shorter version of the Patient Discharge Letter • Nurse uses to review discharge instructions with the patient • Requires signatures of nurse and patient • Is placed in chart as a permanent document • Asthma Action Plan (2 copies) • one copy is given to patient • Other copy is signed by nurse and patient and placed in chart as permanent document • Prescriptions: are still handwritten Next 3 slides are examples of printouts V://Traiining/NEO/MR/2009training/discharge appt scheduling for VCH09.ppt

Medical Receptionist Role Retrieves Documents from Printer and delivers to nurse (prints to same place as HEO/Wiz Orders Print) • Patient Discharge Letter (1 copy) given to pt • Patient Instruction Sheet (1 copy) • Asthma Action Plan (2 copies).. if Asthma identified by Provider V://Traiining/NEO/MR/2009training/discharge appt scheduling for VCH09.ppt
