  2. Responsibilities • Provide adequate supervisory and operational personnel • Pre-approval visits • Annual training • Reviews

  3. Review Frequency Sponsors must review each facility 3 times/year: • At least 2 must be unannounced • At least 1 unannounced review must include observation of a meal service • At least 1review must be made during each new facility's first four weeks of Program operations • No more than 6months may elapse between reviews

  4. Review Elements • Assess if facility has corrected problems from a previous review • 5-day reconciliation of meal counts, enrollment and attendance records

  5. Review Elements • Assess compliance with the following: • Meal pattern • Licensing • Meal counts • Menu and meal records • Attendance at training • Annual updating of enrollment forms

  6. Review Elements • After review, notify day care home of necessary corrective action. • If seriously deficient, notify day care home and the State Agency of deficiencies.
