Definition ( in wiki) • A Health Concern is a health related matter that is of interest, importance or worry to someone, who may be the patient, patient's family or patient's health care provider. • The health related matter is of sufficient interest/importance that someone in the health care environment (patient, family, provider) has identified to be requiring some attention and tracking.

  3. Perspectives • A health concern may be identified from different perspectives: • Patient • Family member/carer • Providers (physician, surgeon, physical therapist, respiratory therapist, nutritionist, health educator, social worker,

Problem concerns • A health concern is felt as having a broader meaning than problem concern, but an exact boundary could not be defined. • The way health and problem concerns will be used will depend on the stakeholders, their environment and the context in which it is used. A problem concern for one personis not necessarily a problem for another or might be a health concern for the other. • This leads to the view that the way health and problem concerns could be used, should be flexible and fluid.

  6. Health Concern tracking • Health concern tracking contains no semantics beyond that need for tracking. • Health concern tracking is used to organize data in the health records. Information like observations, medication, diagnosis and such are grouped into episodes. • This is done by looking back into the past and tagging information to a concern. • This could mean that health concerns are used to personalize views on the medical records.
