Deadline and Delivery Location • All proposals are due NO LATER THAN : • JUNE 26, 2019 @ 3:00 PM • Proposals must be submitted via electronic upload to BuyNet or submit electronic files on CD/DVD to the Front Desk, Department of Purchasing and Contracting, 5560 Overland Avenue, Suite 270, San Diego, CA 92123 • Late proposals are not normally reviewed

California Revenue and Taxation Code • Failure to submit the required forms will result in withholding of payments • Refer to the Franchise Tax Board websites for tax forms and information on nonresident withholding, including waivers or reductions • 590, 587 and W9 • The County will not give you any tax advice. It is recommended you speak with your tax adviser and/or the State of California for guidance

Confidential/Proprietary Information • If proposal contains confidential and/or proprietary information • Submit in a separate envelope marked as • “EXHIBIT – CONFIDENTIAL/PROPRIETARY” • Responses to solicitation requirements shall refer to the response contained within the EXHIBIT – CONFIDENTIAL/PROPRIETARY • Submit an original signed Indemnification Agreement

No later than: 3:00 PM PST/Local San Diego time – JUNE 26, 2019
