Prior to the first day of school • Submit caseloads to Administrators, Counseling, and Health Tech. • Review each case file on your caseload • What was the parent’s prime concern? • Note the qualifying disability • Note present levels • Note any behavioral issues • Note the goals • Review all services: Amount of time that is in special ed classes, speech, OT, APE, Transportation? • Re-read the notes from the last IEP. * Pay special attention to the High Profile Cases

  3. COMMUNICATE HOME • The case carrier needs to call the home of each student on their caseload, introducing themselves to the new parents, and touching base with their existing parents. (be sure to log the contact in easyiep). • Keep the call positive • Let the parent know that transportation pick up and drop off times will be a little off these first few days….don’t panic, we haven’t lost a student yet. • Let the parent know that they should expect some schedule changes the first couple of weeks of school due to classroom balancing etc…. • Ask for any concerns they may have at this time. • Make sure the parent has your contact number and email address.

  4. COMMUNICATE with Administration/Counseling/Nurse • Those students that are high profile, have behavioral issues; behavioral plans and or behavioral goals. • Should campus supervision be aware? • Any Health concerns or medication protocols? • Counsel with the Health Tech. (Keep your district nurse in the loop).

  5. Communicate with General Ed Teachers • Formulate groups within group-wise matching students with their scheduled teachers. • Alert each specific students general- ed teachers about the IEP. • Qualifying disability • Behavioral issues • Present levels (as they apply to the class) • Goals (as they apply to the class) • Accommodations and any modifications • Health concerns and protocols • If the student has a 1:1….what the SCIA’s responsibilities are. • If the teacher wants a copy of the IEP, provide them with “IEP at a Glance.”

  6. FIRST WEEK • Make certain you meet each student on your caseload that first week. (Middle School and High Schools) • Touch base with DIS service providers (Speech, APE, OT, VI and DHH) About those student’s on your caseload who are to receive services. • How is transportation working for those student’s who are to receive transportation services? Any problems that need to ironed out? • Stay on top of any schedule changes as counseling balances classes (This is a good topic to discuss with the parent on your initial call…plus this will affect the student groups created in group-wise). • Begin Scheduling IEP’s (at least 6 weeks out), and sending out Assessment plans as are due.

  7. Management Tools • Due Dates • Use the “advanced” student search in Easyiep to review past dues, and “coming up’s”.

  8. Case Manager’s Checklist • ____ I have submitted my updated Caseload to Administration, Counseling and the Health Tech. • ____ I have Reviewed all case files on my caseload. • ____ I have made phone contact with all parents of students on my case load. Those not available by phone have been sent a introduction letter. • ____ I have identified those students with behavioral / health concerns and notified administration, counseling, or the health tech. • ____ I have obtained my case loads class schedules, created communication groups in groupwise for each student and notified general education teachers of the student’s IEP and needs. • ____ I have contacted DIS service providers with the names of student’s on my caseload they will be seeing, including IEP dates. • ____ I have reviewed any outstanding/carryover assessment plans needing immediate action. • ____ I have scheduled all IEP’s needing immediate action. • ____ I have scheduled my next set of IEP’s due at least 6 weeks out.

  9. Review of the EssentialFirst 14 days • Case carrier needs to familiarize and re-familiarize themselves with each case file • Case carrier needs to make first contact with the home • Case carrier needs to communicate with site administration, counseling, DIS Service providers and health techs. • Case carrier needs to meet with each student on their case load. • Case Carrier needs to communicate with general education teachers. • Case Carrier needs to begin planning IEP team meetings.
