  2. 1. Objectives • To undertake visit for monitoring NDD or MUD using google monitoring form (February 2019 round) • https://docs.google.com/forms/d/e/1FAIpQLScXP-aIO1tBCjGn1xZIkUfpQL-aiLCwMNttotSXks9M9l_IEw/viewform • To visit allocated number of schools & AWCs for same Laptop view Mobile view Google Forms aligns itself as per the screen size

Filling of Monitoring Form • All questions are mandatory. • If AWC is located in a school premises, write specifically name of that AWC and not school’s name. • Ensure mobile number should be 10 digit. • Enter complete DISE code of schools & AWC code properly & carefully. • Take sufficient hard copies of the monitoring checklist. • Fill hard copy forms in case of internet issues. Fill this information on google monitoring form on availability of internet.

Entering Data in Google Monitoring Form • Total attendance should be less than or equal to registered/enrolled students, in case of school/technical institutes/colleges/ITIs etc. • Registered/ enrolled students and total attendance column is not applicable in case of AWC, hence mention “Not applicable”. • Specify the quantity of drug in both the cases i.e. if available in sufficient quantity or insufficient quantity. • Select date of expiry of drug with the help of inbuilt calendar in the google form. • The date will appear in MM-DD-YY format. • Do remember to specify/elaborate, if ‘’other’’ is selected in multiple choice question.

Entering Data in Google Monitoring Form (Contd…) • If Q4 is “Yes” then in Q5 write “NA” in other option. • Select option “NA” in Q 7, Q 8, Q 10 and Q 11, if monitoring site is school. • Last section on “Additional Comments” should capture only suggestions for program improvement and feedback on any program gap and best practices observed on NDD/MUD. • In Q18 is “Yes” then in Q19 write “NA” in other option. • Avoid using comments like very good/excellent in this section.

  6. 4. Data Submission • Before submitting the data, please ensure the following: 1. Follow the same style of writing the name of monitoring officer e.g. write full name with using abbreviations i.e. Ravi Kumar instead of R. Kumar or Ravi K. 2. Please don’t write title before monitor’s name e.g. Write Ravi Kumar instead of Dr. Ravi Kumar 3. Ensure the same spelling of monitor’s name, state/district/block/urban area/village/ward name • Click submit button after cross checking. • On successful submission of data pleasedo not choose back option. Instead, select “submit another response” displayed on mobile/Web screen.

  7. 5. Other Important Points • Every monitor should carry the hard copies of NDD monitoring checklist as a backup to cope-up with any internet/network issue. • Charge your phone before you proceed with visits. • Check if link is working or not.
