  3. 1998 • 2 Sep 98 - Swissair wide-bodied MD-11 • Cause Unknown - Currently under investigation. (Electrical wiring fire in cockpit is suspected) • 16 Feb 98 - China Airlines Airbus • Weather related (heavy rain and low ceilings) and procedural tasks errors on the part of the flight crew. Three approaches attempted! • 2 Feb 98 - Cebu Pacific Air DC-9 - • Weather related and failure of the flight crew to follow established instrument procedures (altitude related) during approach phase

  4. 1997 • 19 Dec - Singapore SilkAir Boeing 737-300 • Unknown Cause - possible suicide of Captain - Still under investigation. • 26 Sep - Indonesian Airbus A-300-B4 • Flight crew procedural task errors during instrument approach procedure. • 6 Aug - Korean AirLines flight 801 • Flight crew procedural error during instrument approach at night and in heavy rain.

  5. 23 Nov - Ethiopian Airlines Boeing 767 Hijacked commercial airliner - fuel exhaustion 12 Nov - Saudi Arabian Jumbo Jet Worst-ever mid-air collision. ATC Human Error event. 7 Nov - Nigerian Boeing 727 Cause - Sabotage 29 Aug - Tupolev 154 Cause Unknown - probably mechanical. 1996

  6. 1996 • 17 Jul - TWA flight 800 • Electrical wiring in the center fuel tank pump overheated and ignited vapors resulting in an explosion and loss of control. • 10 May - ValuJet DC-9 • Illegal cartage of flammable oxygen generators in baggage area that ignited resulting in fire and loss of control.

  7. 1996 • 29 Feb - Boeing 737 • ATC and flight crew procedural errors during Instrument Meteorological Conditions. • 6 Feb - Dominican Alas Nacionales, Boeing 757 • Maintenance related - Tape was painted over static ports resulting in wrong airspeed read-outs and loss of control. • 8 Jan - Russian Antonov-32 Cargo Plane • Mechanical related cause - powerplant failure - loss of airspeed.
