Care and Support Planning – My Support Broker • What is the Support Planning - MSB pilot project • Focus on broader life outcomes – more choice and control • Builds on information from the assessment • Trained 5 x support brokers and 2 x principal brokers who facilitate self-directed support plan • Support Plan details the needs, how they are met and how they link to the individuals desired outcomes and to the wellbeing principle • Who are My Support Broker – partner organisation provide accredited Support Planning training, support cultural change, start pilot • Benefits being realised- Better use of social capital, different skill mix in care planning (specialised brokerage), increase in Direct Payment take up

Progressto date • Phase 1 until Aug 17 will gain learning/benefits from pilot to upscale brokerage capacity and develop a new social care pathway • Brokers completing training and have begun live planning • Will be examining a personalised social care pathway and implementing this across 25-65 workforce

  4. Implications for social care market • Anticipate people will want to commission more flexible and tailored services • May be implication for some block contracts if there isn’t continued demand • More flexible ways of contracting • Use of ISF – Individual Service Fund midway between DPS and council contract with a commitment to pay provider in advance service user chooses when to draw down e.g. respite

  5. Implications for social care market • Expand number of personal assistants (PA’s) • Improve accreditation of PA’s • Providers will have to start advertising services to brokers in a more tailored and individual basis menu of services • Anyone like to pilot an ISF with us? • Here to get feedback and ensure keep providers updated on pilot progress
