Hi Trust: Status Update 10.03.18 • Timetable: • October 15thHealthix submits final evidence to assessor • December 14th assessor certifies Healthix • Certification requires each domain to score: • 71% or higher, or • 62%-70% with a Corrective Action Plan • Each QE must attain HITRUST Certification by December 31, 2018 to retain funding. HIV AIDs Alerts - Current & Future Grants Out of Care • Deliver alerts when a consentin patient is out of care and not virally suppressed at last lab. • Alerts stratify patients by CD4 count to facilitate prioritized outreach Grant #01 • CURRENT STATUS: • Development Complete. Scheduling testing with pilot site. • Projected Go-Live: Early November LTC - HIV Case-finding algorithms Linkage To Care • Identify HIV+ patients who have been lost to care, using diagnosis and lab results. • Deliver real-time alerts when the LTC patient registers at an ED. • CURRENT STATUS: • Pilot site went live on 09/19/18 • Project retrospective in progress Grant #02 At Risk for HIV • Predict the likelihood of contracting HIV AIDs in high risk individuals and deliver patient-specific real-time alerts to care providers, frequent reports to care managers and public health agencies for appropriate intervention. CURRENT STATUS: - Application pending approval. Grant #03 Average time for alert to be delivered = 49 seconds
