  3. What is the purpose of panel identification? • Panel identification is the first step in creating the medical health home • Eliminate unnecessary information by “thinning” the electronic chart • A well identified physician panel allows for progress into panel management

  4. Panel Identification Creating a solid foundation for the patient medical home What is panel identification? There are 2 components to panel identification: • Updated information within the patient record including the “most responsible physician” • The ability to generate a physician panel list This process is not physician work it is teamwork!

  5. Patient panelling is not new to the physician clinic. The onset of EMR (electronic medical records) has just changed the way this work must happen. The EMR is all these systems and more right at your desk! Primary physician’s still need to be identified Inactive patient, non-clinic, & deceased patient status needs updating Lists and forms are built into the system Search parameters combined with accurate demographics allow advance patient care planning Imagine the doctor’s office in 1995………. • Coloured file folders may have been used to differentiate the primary physician • Tags on folders might indicate the year of last visit and a patient without a visit in 3 years was moved to a different shelf • Deceased patient files may have been moved to storage • Checklists on the inside cover could be used for screening • You knew the charts were up to date

  6. What is panel management? Dr. Ifeelfine has been leading his clinic team to implementation of panel processes. The patient status was updated including deceased and non clinic patients. The demographics are being confirmed at least every 6 months and the roomers are ensuring patients identify a primary physician with most recent visits. He feels ready to take advantage of the success with panel management and screening! On the spot screening(during office visits or when booking) He has brushed up his EMR rules and ASaP(Alberta Screening and Prevention) knowledge. With upcoming appointments he will ensure patients receive requisitions for overdue labs and tests. He will have his MOA assist. Outreach Screening He is concerned about patients overdue for mammograms. His team lead generates a list of female patients born in June, with no mammogram results in the past 2 years. The list is used to create a letter and requisition to be mailed to the patients……18 month later the EMR makes the list!
