  4. What did we do in Fife? • In October 2009 as part of the demonstrator project in Intermediate Care we decided to use the IoRN as an outcome measure for the duration of the project in teams across Fife with Health and Social Care staff

  5. Intermediate care services are defined in Fife as The concepts of “preventing unnecessary admissions to hospital” and “supporting discharge from hospital” that can be “provided by a variety of services including health, social care and housing” (Gilmour M; Valentine JA; McLaren G, 2009)

Conclusion From the results of the demonstrator project in Fife The IoRN was able to give results that showed a difference when the tool was used at the beginning and end of and episode of care The IoRN enabled us to have clear team profiles The IoRN gave us an overall picture of the possible effects of Intermediate Care services
