How does the Policy Work? • Pays a Lump-Sum Cash Benefit • $20,000 for the employee • $10,000 for spouse/child/children • These amounts are for major events (i.e. Heart Attack, Stroke, Cancer and Major Organ Transplant (See Product Details in the Brochure for complete list) • There is a maximum payout of 3 x the benefit amount

  14. Recurrent Benefit50% of the first occurrence amountExample: Employee has a heart attack and survives. Files the claim and receives a lump-sum for $20,000.Two years later has another heart attack and survives. Files the claim and receives a lump-sum for $10,000.For separate event or diagnosis, insured receives full amount for first occurrence based on the schedule (See Product Details in Brochure for list)

  16. Wellness Benefit Included$150 per insured per yearExample: Employee/Spouse coverage. Employee is in the 30-39 age bracket. Their annual premium is $452. Each has their yearly physical, files the claim with Transamerica and each will receive $150 in wellness benefit for a total of $300.

  18. How to submit Application • The application can be completed on the web portal or you can print and complete • Effective Date will be the first of the next month if completed by the 22nd. If completed after the 22nd, it will be effective the first of the following month. • Complete employee information • Complete dependent information if you are covering spouse, children, or both • Complete benefit selection, employee only, one parent family or two parent family and input rate • Rates are based on the employees age bracket, one rate only • Sign and Date • Print, Scan, and or fax application to: Anna Dempsey adempsey@alfains.com
