  2. Survey Week – what we did • Attempted to survey everyone on streets, in shelters and at drop-in service centers • Gave them optionto completeVulnerability IndexSurvey

  3. Survey Week – what we did • Thursday & Sunday (prior to Survey Week): • training on Vulnerability Index • Monday – Friday: • mid-morning, afternoon andevening outreach and shelters • Tuesday & Wednesday: • 4-6 AM surveyed streets

  4. AM Interview Methodology • Team leader with 3 or more team members • Divided Albuquerque into sectors • Covered sectors on two consecutive mornings • Asked each person encountered to participate • Same methodology was used in 2011

  5. Vulnerability Index • History of Cold Weather Injury • 3 or more ER visits in the previous 3 months • 3 or more ER and/or inpatient episodes in the past year • End-stage Renal Disease • Liver Disease • HIV+/AIDS • Age > 60 years • Tri-morbidity

  17. Future Success:Apartment Association of NM • “Home to Home” • People can add to their monthly rent • 100% goes to help someone gainhousing • Goal: House 5 people in first year(raise $32,500)
