  3. To (Arrival airport): 3 letters as per IATA code

  4. Flight number as per VNA flight plan

  5. Captain name

  6. A/C type IAW 3 character as per VNA requirement: AT7, 777, 320, 321, F70

  7. Aircraft registration Number

  8. Date: dd-mm-yy

  9. Delay more than 15 minutes

  10. Delay time = Chock off-scheduled time (xx hrs yy mins)

  11. Write down the technical Delay Code Delay Code table is attached to Tech log for reference. For other delay reason (by non-technical reason or combined reason), Delay Code is not required.

  12. Write down the 3 letter code of abnomal flight that is attached to Tech. log book.

  13. Defect raised by Pilot or maintenance staff, if not, state NIL FURTHER

  14. Item: Sequence number of defects/ technical status. It linked to item number of part replacement details

  15. ATA code as per ATA 100 (At least 4 digits)

  16. Report by ENG or CREW, Cross (X) in appropriate blank

  17. Captain signature for all informations given above by him and also for remaining fuel given below

  18. Action taken: describe maintenance action for reported defect rectification

  19. Item: Sequence number of reported defectsand/or part replacement (if any)

  20. Acceptable Deferred Defect sequence number as registered in ADD lists including MEL and Non-MEL related Item

  21. Number of the ADD being cleared (fill clearance info. in ADD list as well)

  37. Discrepancy = Total uplift- Bowser uplift MMI sticks shall be used to check if discrepancy is higher than 3.5%

  38. Fuel bill number of refueling

  39. Oil /HYD service: Recording added and total quantities, if not state “NIL”

  40. CRS staff certifies the fuel and oil quantities and distribution

  43. APU hours and cycles recorded when required
