  2. What is LTSS? Long-term services and supports (LTSS) are a spectrum of health and social services that support elders or people with disabilities who need help with daily living tasks. LTSS can be provided in the home and community or in a facility. • Medical, personal, and social services • Help people with functional limitations, including elders and people with disabilities • Home- and community-based services are preferred option to maximize independence

Types of LTSS Services Caregiver and client training Case management Dietary management by dieticians Disease prevention Durable medical equipment Health promotion Hospice care Medication management Occupational therapy Pharmacy Personal emergency response systems Physical therapy Respite care for caregivers Skilled nursing Speech therapy

Adult day care or senior centers Financial management, help with bill-paying Home-delivered meal programs Home safety assessments, repairs, and modifications Homemaker and chore services Nutritional meals at congregate sites Information and referral services Legal services Personal care (dressing, bathing, etc.) Telephone reassurance Transportation and access

  5. LTSS RoadMap For tribes interested in establishing an LTSS program in their communities. Designed to help people understand their communities needs.

  6. Step 1: Needs Assessment

  7. Step 1: Needs Assessment Collecting of information that provides a complete picture of your community. Helps identify and prioritize needs of the community. A crucial step in the roadmap process

  8. Steps to a Needs Assessment Identify initial needs Collect Data Define LTSS Needs Describe LTSS Needs

  9. Step 2: Goals & Objectives Goals set your purpose and state which issues you want to address. Objectives are measurable targets that help you meet your goal. Performance metrics are ways to measure changes in your community, to see if you are meeting your goals.

  10. To help identify your goals and objectives, consider these questions: What goals do you want to achieve through your LTSS program? What are some objectives you need to accomplish to help achieve the goal?

  11. Step 3: Compare Models LTSS programs follow one of two models: Home and Community-Based Services Facility-Based Services Financial Considerations and the level of tribal involvement in LTSS program delivery differ between models.

  12. Step 4: Select a Model Study each model to understand how each relates to your community, consider the following: Impact Required Resources (Non-Financial) Sustainability and Cost Effectiveness

  13. SWOT Analysis 1. Hold a meeting of people who know about LTSS, health care, elder care, social services, and Medicare and Medicaid services and programs. 2. Create a list of strengths, weaknesses, opportunities, and threats for your LTSS project. 3. Review and discuss the items in each of the four categories, ensuring items are in the correct categories. 4. Develop action items that deal with each quadrant of the SWOT matrix. Examples of action items include: • Strengths: How can we take advantage of each strength to expand or build LTSS? • Weaknesses: How can we address and correct these weaknesses? • Opportunities: How can we use these opportunities to benefit our program? • Threats: How can we reduce the risks posed by these threats?

Step 5: Planning Strategic planning can: Give you a clear understanding of your community’s current situation Help you anticipate problems Build on your community’s current strengths Steps to Strategic Planning: Understand your LTSS model • Review program requirements, identify your funding source, figure out amount of financing Plan strategically • Identify things that might affect your program Prepare for implementation • Create a implementation plan and develop timelines, line up your resources

  15. Step 6: Implementation The implementation checklist can help you get a clearer view of certain aspects of your program. Implementation Checklist: • Financing • Accreditation • Buildings • Staffing Certifications Develop a Work Plan Strategies for success • Existing resources, communicating with key stakeholders, build and use partnerships.

  16. Step 7: Evaluation Evaluation measures your progress towards your goals and objectives, helps identify successes and areas that need improvement. Evaluation Questions Key Elements Logic Model Evaluation is a continuous process. Use your evaluation results to help improve your program.
