  12. Skills Vital signs: HR, BP, Temperature, RR Blood Pressure: normal range- systolic:100-140 diastolic: 60-90 Ex: 120/60 Use correct size cuff Make sure the cuff is in the correct position Do not over inflate the cuff

Skills Vital signs: HR, BP, Temperature, RR Heart Rate: the number of times the heart beats in 1 minute Normal range: 60-100 Use radial/brachial pulse sites and count for 1 full minute OR listen with a stethoscope Also note regularity when checking pulse(regular/irregular) Notify professional if abnormal value obtained(recheck if appropriate)

Skills Vital Signs: HR, BP, Temperature, RR Normal: 98.6 F Hypothermia: 95F or below Fever: 101F or greater Temperature- prefer oral temperature, may take axillary temperature if pt. has had food/drink within prior 15-20 min. Make sure you are using proper technique when taking temperature Record temperature and where temperature was taken

  15. Skills Vital Signs: HR, BP, Temperature, RR Respiratory Rate: 14-18 breaths per minute Breathing in and out= 1 breath Note breathing pattern Count respirations for one full minute

  16. Skills Pulse Oximetry: measures oxygen concentration of blood Normal: 92-100% Device place on finger/ red light must be placed over fingernail Not accurate if pt is moving around or is on the same finger as taking blood pressure. Must correlate with the heart rate in order to be accurate

  17. Reporting Report all client assessments and observations to a professional or site proctor Use black ink only- NO PENCIL Write neatly Always report abnormal values or changes verbally Cross mistakes out with a single line and initial(do not use white out or scribble over mistake) Notify professional/site proctor if you need to step away for a moment

  18. Infection Control Wash hands: 20-30 sec. of lathering Before and after going into a patient’s room Before and after eating and using the restroom Before and after any patient contact Regularly throughout the day Always use PPE(personal protection equipment) gloves, gown, mask, eye shields when necessary Use hand sanitizer only when hands are not visibly soiled

Privacy Privacy and Confidentiality: Do not share any information obtained in clinical about a patient outside of clinical Do not use the patient’s name on any paperwork leaving the hospital If a patient is someone you know outside of clinical; do not tell anyone you saw/worked w/that person and keep info. confidential Respect patient’s privacy: knock on the door, close the door after entering, close bathroom door if it is safe, allow patient to bath his/herself if able/safe
