  1. Junior Program Enrollment Form Grades 7 & 8

What is Junior School Counts!? • For many middle school students, making the connection between what they learn in the classroom and what employers expect on the job can seem like an impossible task. • Junior School Counts! helps students make that connection and possibly get them on a track to earn a college scholarship too!

  3. Who is eligible? • All Grade 7 & 8 students. If you already enrolled in Grade 7, you will not need to enroll again. If you cannot remember, go ahead and submit a new form. • Seventh and eighth grade students who meet the criteria will receive a Junior School Counts! certificate at the end of each school year.

  4. Why participate? • Participation in Junior School Counts! Helps prepare students for School Counts! At the high school level. Like its junior version, School Counts! Rewards students for punctuality, good attendance and good grades. • When School Counts! Students achieve the criteria standards every year for all four years of high school, they are automatically eligible for a full tuition scholarship to Salem Community College!

  5. The Criteria: • Students enrolled in Junior School Counts! must adhere to the following criteria: • Be enrolled as a 7th or 8th grade student • Earn a grade of “C” or higher in every course • Achieve a 95 percent attendance and punctuality record (95% is attending 171 school days or having 9 unexcused days absent)

To fill out the form: • Name • Address • City, Zip Code • Birth Date • Home Phone Number • School Name: Woodstown Middle School (grades 7-8) • I am currently a: (circle one) 7th Grader 8th Grader • Student Signature, Date • Parent/Guardian Signature, Date • Mrs. Nelson’s Signature • Pink copy will be returned to you when complete

  7. When are forms due? • Completed forms are due to Mrs. Nelson by Tuesday, October 15, 2013if interested. • Parent or guardian signature is required.
