第十节 臀部、股后区. 一、 皮肤切口. 1 ．臀股沟作一横切口。 2 ．腘窝横纹作一横切口。. 二、 解剖程序. 1 ．翻开皮肤。 2 ．浅筋膜内找： （ 1 ）臀上皮神经 （ 2 ）臀中皮神经：又名臀内侧皮神经 （ 3 ）臀下皮神经 （ 4 ）股后皮神经. 3 ．清除浅、深筋膜。 4 ．分离臀大肌，切断起点，翻开。 5 ．清理臀大肌下结构： 1) 臀上神经、动脉、静脉 ( 外侧→内侧排列，与股神经、动脉、静脉同 )→ 梨状肌上孔。 2) 坐骨神经、股后皮神经、臀下神经、动脉、静脉、阴部内动脉、静脉、阴部神经

  2. 一、皮肤切口 1．臀股沟作一横切口。 2．腘窝横纹作一横切口。

  3. 二、解剖程序 1．翻开皮肤。 2．浅筋膜内找： （1）臀上皮神经 （2）臀中皮神经：又名臀内侧皮神经 （3）臀下皮神经 （4）股后皮神经

  4. 3．清除浅、深筋膜。 4．分离臀大肌，切断起点，翻开。 5．清理臀大肌下结构： 1)臀上神经、动脉、静脉(外侧→内侧排列，与股神经、动脉、静脉同)→梨状肌上孔。 2)坐骨神经、股后皮神经、臀下神经、动脉、静脉、阴部内动脉、静脉、阴部神经 (外侧→内侧排列) →梨状肌下孔。 3）阴部内动脉、静脉、阴部神经(外→内)→坐骨小孔。

  5. 6．切断臀中肌，翻开，观察臀小肌、臀上神经、动脉、静脉。6．切断臀中肌，翻开，观察臀小肌、臀上神经、动脉、静脉。 7．观察上孖（zi或ma）肌、闭孔内肌、下孖肌、股方肌(上→下排列)。 8．清理股二头肌、半腱肌、半膜肌。 9．查看坐骨神经的行径、分支和穿动脉(股深动脉的分支)。
