Nypro (UK) chemical plant in Flixborough 1974: Cyclohexane released from a by pass installed to connect reactors either side of a faulty reactor that had been taken out of service. Disastrous explosion destroyed the chemical plant 28 workers were killed, a further 36 suffered injuries Extensive damage to property off site Advisory Committee on Major Hazards appointed by HSC, who proposed a three-part strategy: Identification of the sites; Control measures to prevent major accidents; and Mitigatory measures to limit the effects of any accidents which do occur Seveso, Italy 1976: accidental production and release of a dioxin, an unwanted by-product from a runaway chemical reaction, led to widespread contamination. 3,000 pets and farm animals died. 70,000 animals slaughtered to prevent dioxin entering the food chain. 193 people suffered symptoms. EC proposed a Directive on the control of major industrial accident hazards.
