Yale Fatality • Raymond Clark III, a lab technician accused of killing 24-year-old Yale graduate student Annie Le. • Le's body was found Sept. 13 concealed in a crawl space at 10 Amistad St., a research building that is part of the Yale School of Medicine complex where both Clark and Le worked.

  3. Solvent Spills • Chloroform • Researcher carrying 4-L bottle in bucket from store room to laboratory • Bucket disintegrated, smashing bottle in interior hallway (lab suite) • DEHS notified (almost) immediately • Ethyl Acetate • Researcher wheeling 4-L bottle in bucket on cart from store room to laboratory • Door bumps cart, launching bottle out of bucket, off the cart, smashing onto floor of public hallway • DEHS notified when researchers several floors above the spill notice the smell

Fume Hood Fire • Hotplate/stirrer being used for reflux rxn in fume hood. • At the end of rxn, instead of turning hotplate off, dial turned to high, researcher returns to office • Mineral oil on hot plate sprays over the inside of the hood causing multiple areas of burn. • A second researcher uses 2 extinguishers on fire

UCLA Lab Fire • 22-year-old Sheharbano Sangji hired in October, 2008 as a research assistant at UCLA • safety inspection by UCLA that same month identified various deficiencies in her PI’s labs, including failure to document training and failure of personnel to wear proper protective clothing. • On Monday December 29, 2008, Sangji intended to withdraw 20 ml of a 1.7 M solution of tert-butyllithium (t-bu-Li) in pentane into a 60 ml syringe • Syringe came apart, chemicals contacted her gloves and clothing and ignited. • Sangji walked passed emergency shower • Sheri passed away on January 16th, 2009
