Establishing an IBC HSA-qualified High Deductible Health Plan and Bancorp HSA Independence Blue Cross has a preferred relationship with The Bancorp Bank to provide HSA services. 1st Step: Enroll in an IBC High Deductible Health Plan 2nd Step: Enroll in a Bancorp Health Savings Account Enrollment in a Bancorp HSA can be processed through the ibxpress portal if a portal customer or by application available through IBC 3rd Step: Enrollment is reviewed by Independence Blue Cross (IBC) 4th Step: For members that have elected to enroll in a Bancorp HSA (through group portal selection or paper application), IBC transmits demographic data electronically to The Bancorp Bank Bancorp receives enrollment data, and immediately reviews information within 24 hours. The HSA is opened after validation according to Customer Identification Procedures outlined in The USA Patriot Act 5th Step: Bancorp opens a Health Savings Account for the IBC member

Establishing an IBC HSA-qualified High Deductible Health Plan and Bancorp HSA (continued) Within 2 weeks, the following items are mailed to each new Bancorp HSA account holder: Welcome Kit – including User Id (mailed in large white envelope) Password for Online Access (Mailed in business envelope, or may be sent electronically if email is provided) PINfor Debit Card Cash Access (mailed in small perforated mailer) Checks– Book of 50 (mailed in a blue plastic envelope) Visa Debit Card (Mailed in a regular business envelope) 409 Silverside Road, Suite 105 Wilmington, DE 19809 NOTE: After Bancorp account opening, be sure to watch for new materials in the mail. For security, account-related materials are mailed in generic envelopes simply noted with a return address of: 409 Silverside Drive, Suite 105 Wilmington, DE 19809
