  2. LABORATORY HYGIENEPROGRAM • Does the lab have an unkempt appearance? • Is there a protocol for lab equipment cleaning and maintenance? • Are tables, equipment, etc. disinfected according to established protocol? • Are chemicals, equipment, and other materials kept in secured conditions?

  3. LABORATORY HYGIENEPROGRAM • Has a Hazard Analysis been conducted and is kept current? • Is the CHP reviewed and evaluated annually? • Is the CHP available and accessible? • Have all employees been trained on the details of the CHP? • Are there Uniform Lab Hazard Signs at each entrance?

LABORATORY HYGIENEPROGRAM • Is there a clearly marked evacuation route in relation to the work area? • Are there emergency #’s (7747) stickers on the phones and posted in other conspicuous places? • Is the Chemical Inventory List (CIL) current? • Are MSD Sheets readily available to students and employees?

  5. LABORATORY HYGIENEPROGRAM • Are all containers properly labeled? • Are Peroxide formers present? • If yes, are they dated at the time of purchase? • Are chemicals stored by hazard class?

  9. LABORATORY HYGIENEPROGRAM • Are flammable liquids stored in a refrigerator? • If so, is the refrigerator suitable for flammable solvent storage? • Are food prohibition regulations adhered to?

LABORATORY HYGIENEPROGRAM • Are respirators available? • If yes, are personnel fit-tested and trained? • Is eye protection worn at all times? • Are appropriate gloves worn as required? • Are necessary safety shields used? • Is appropriate footwear used as required? • Are aprons and/or lab coats worn as required?

LABORATORY HYGIENEPROGRAM • Is eating, drinking, smoking, and applying cosmetics prohibited in the lab area? • Are aisles clear and unobstructed? • Is there a spill kit in the lab? • Is a hazardous waste collection system used? • Are employees and students trained in hazardous waste collection?

LABORATORY HYGIENEPROGRAM • All electrical equipment is safely grounded? • Are electrical cords in safe condition? • Does the lab avoid the use of extension cords as permanent power supplies? • Are circuit breakers unobstructed and no flammable substance is placed within three feet of the panel?

  13. LABORATORY HYGIENEPROGRAM • Does the lab have a safety shower? • If not, is there a need for one? • If yes, is it within code? • Was it inspected within the last six months? • Is it clearly visible and accessible? • Is it provided with a modesty curtain?

  15. LABORATORY HYGIENEPROGRAM • Are there any significant and unpleasant odors in the lab? • Are there unusual or loud noises in the lab? • Is there a fume hood in the lab? • If so, is it inspected at least annually? • Is the fume hood used for storage?

LABORATORY HYGIENEPROGRAM • Are there Uniform Lab Hazard Signs at each entrance? • All doors that are intended to be access doors are accessible. • Doors that appear to be exit doors, but aren’t, are properly labeled (basement, storage, no exit, etc.) • When people are present, exit doors are never locked, blocked, or chained. • An exit sign can be seen from wherever a person might be located.
