Referrals • Pathways and Move-On Team (PMOT) work closely with Private Sector Initiatives Team (PSIT) both based in Housing Options Service • Close links with providers • Rights and responsibilities of being a private tenant in support planning • Very clear referral criteria (see handout) • Ultimate decision is with PMOT

  3. Landlords • PSIT ensures landlords aware of the background of clients (confidentiality) • Landlords aware of processes (PMOT and floating support) • Dedicated Tenancy Sustainment Team within PSIT – 2 weeks, 5 month, 11 month and 35 month checks

  4. Support and sustainment • Floating support via one provider (OHG) • 3 way meeting – engagement • Attends viewing and tenancy sign-up • Partly based in PMOT • Issues/closure/disengagement fed back to PMOT and tenancy sustainment (PSIT) VERY IMPORTANT • Floating support worker promotes the landlord/tenant relationship – does not become landlord’s point of contact
