Kentucky Health Information Exchange • The Kentucky Health Information Exchange is the name given to the information system that will support statewide exchange of health information among healthcare providers and organizations according to nationally recognized standards. The state of Kentucky was awarded a grant in late 2009 to develop the KHIE and as part of the pilot effort Central Baptist Hospital will be evaluating this application. • Access to KHIE is provided to authorized users via the Baptist Intranet (BEN). The authorized physician or clinician can query KHIE to provide a Continuity of Care Document (CCD) based upon information from Kentucky’s Medicaid system. The physician/clinician can query the KHIE database using three different search possibilities: • The patient’s Medicaid Number. • The patient’s Social Security Number • The patient’s Name and Date of Birth • If a match is found, then the data for this patient will be displayed on the CCD. The header on the top of the CCD will identify the patient and the Table of Contents shows the types of data available on the CCD. At this time based on the feed of data from Kentucky Medicaid, you will see data in the following sections: • Problems • Medications • Procedures • Encounters

  6. Step 1 – Accessing KHIE from BEN • From the Baptist Intranet Site (BEN), select Login in the upper right hand corner.

  7. Step 2 • After logging in and selecting the Applications Tab, you will see under the ‘My Applications’ box in the lower left-hand corner the selection Kentucky Health Information Exchange. Please note that what is listed in the My Applications box is different for each user based upon what applications you have authorized access to. The applications listed in this section are listed in alphabetic order.

  8. Step 3 • Click the Kentucky Health Information Exchange link and the following screen will be displayed. • You can now look up a patient by using either the patient’s: • Medicaid ID • Social Security Number (SSN) • Last Name, First Name and Date of Birth – please note that birth month and birth day must include the leading zero if needed.

  9. Step 4 • The Continuity of Care Document (CCD) will now be presented.
