  2. New Admissions • Complete history and physical • Please remember birth history if less than 5 or relevant • Remember measurements • Admission Orders • Review admission orders for completeness and clarity

  3. Admission Orders • Admit to /Attending • Diagnosis • Condition • Vital signs/monitoring (standard is 4 times daily) • Activity • Allergies • Nursing - NG tube, wound care, etc…

  4. Admission Orders • Diet • IVF/Oral hydration • Medications • Studies • Labs

  5. Medication Orders • Ampicillin 500mg IV q6hr • Paracetamol 150mg PO/PR q4hr prn fever dose interval med route prn and reason

  6. Common Medication Dosing • Ibuprofen 10mg/kg PO q6hr (prn fever/pain) • Paracetamol 10-15mg/kg PO/PR q4hr (prn fever/pain) • Amoxicillin 40mg/kg PO daily DIVIDED q8hr • Ampicillin 150-200mg/kg IV daily DIVIDED q6hr** • Gentamicin 7.5mg/kg IV daily** ** dosing schedule is different in neonates

  7. Daily Note • Talk to the parents (especially regarding eating, drinking, voiding) • Review nursing notes • AM vital signs • Trend vital signs for 24 hrs • Present plan by problems • Give doses in ‘per kg’

  8. Pediatric Discharge Criteria • Drinking well (+/- eating) • Afebrile x 24 hrs • Tolerating PO medications
