How it started… • QLD Health engaged GPQ to undertake a project to assist the Queensland Clinical Senate in developing a policy position on GP Liaison roles. • Purpose: • To review the roles and skill requirements of the current GPLO workforce to improve the GP – Hospital interface • Develop a statewide policy framework and implementation plan for ‘GP Liaison’ • Steering Committee established

  3. Using an Evidence-Based Approach • Literature review evidence: • The use and impact of GPLOs working to improve the interface • Barriers • Enablers • Practice skills of current GLO workforce • Semi-structured interviews (35 participants)

  4. What was developed? • GPLO model and scope of practice Including: • Principles • Standards for integration • How it would integrate with national health reform • Role descriptions and scope of practice • KPIs • Sustainability • GPLO implementation plan for Queensland

  5. General Practice - Hospital Integration Unit (GP-HIU)

Breakdown of the Model… • Team-based strategy for positioning the roles as key integration ‘boundary spanners’ • Two types of roles undertaken by GPLOs: • Strategic systems level role: General Practice-Hospital Integration Specialist (GP-HIS) • Activity based role: General Practice-Hospital Integration Practitioner (GP-HIP)

  7. Breakdown of the Model… • Project Officer (Primary Care) • Based in a Medicare Local • Provides administration support to GP-HIS and GP-HIP • GP Liaison (Hospital Based) • Support from the hospital sector • Identifies opportunities to improve interface
