  4. ABCD III: Why linkages and care coordination? • Effective developmental interventions generally involve more than one provider or system of care • Various types of providers offer developmental screening and need to make appropriate referrals • Young children often fall through the cracks

  5. ABCD III levels of intervention • Primary care practices (e.g. care coordination protocols) • Community linkages (e.g. common referral and feedback forms) • State systems (e.g. data system linkages)

  6. ABCD III strategies • Developing consistent instruments, tools and processes • Using provider incentives • Implementing continuous improvement strategies • Linking data systems • Improving systems • Building consensus and/or shared visions to promote spread and ensure sustainability
